| Graduating from University of Illinois School of Dentistry in 1980, Dr. Rita Kapmarski has been in private
practice for over 27 years in the Chicago area. She is a member of the ADA, DOCS, Illinois State Dental
Society, Chicago Dental Society, American Association of Dental Consultants, Academy of General
Dentistry and the American Dental Society of Anesthesiology. She has participated in advanced dental
education courses at Harvard University, Northwestern University, and University of Chicago to name a few.
We were awarded 'One of the Best in CHICAGO Dentists 2007' by Chicago Magazine. Member of
HEALTHCARE AMERICA REACHOUT which provides dental services for our troops.

| Dr. Choi completed her undergraduate work at the University of Illinois, at Chicago with a Bachelor of
Science degree in biology. In 1999, she received her DMD from Southern Illinois University School of
Dental Medicine. She finished her specialty degree in periodontics at St Louis University, in Saint Louis,
in 2002. During her periodontal training, she worked as an associate professor for 2 years at SLU, and
received the first award for excellence in implant dentistry. She is a member of the American Academy
of Periodentalolgy and speaks fluent Korean.

| Dr. Washington completed her undergraduate studies at Albion College with a bachelor of arts degree in
Biology. She later attended the University of Michigan School of Dentistry where she received her DDS
in 2001. In 2005, Dr. Washington completed a two year orthodontic residency and received a certificate
in Orthodontics from Howard University College of Dentistry. Dr. Washington is a member of the American
Association of Orthodontists, American Board of Orthodontics, Chicago Dental Society, and the Illinois
Dental Society. |
